In Home Habilitation

Our Approach

Every individual we serve has unique strengths, goals, and preferences. Our approach is built on person centered planning, where the individual and their family are at the center of every decision. We do not believe in one size fits all care. Instead, we create customized support plans that evolve as needs and aspirations change.

Person Centered Planning

We begin with a comprehensive assessment that explores the individual’s goals, interests, daily routines, and support needs. Our team works collaboratively with the individual, their family, guardians, and any existing providers to build a plan that honors autonomy and promotes growth. Plans are reviewed regularly and adjusted as goals are achieved or circumstances change.

Trained, Compassionate Staff

Our direct support professionals receive ongoing training in IDD specific care, positive behavior support, communication strategies, and health and safety protocols. Many have years of experience supporting adults with autism, Down syndrome, cerebral palsy, and other developmental disabilities. We match caregivers carefully to ensure personality compatibility and consistency.

Safety and Peace of Mind

All staff are background checked, screened, and trained in emergency response. We use electronic visit verification (EVV) to ensure accountability, and families receive real time updates through our communication portal. Nurses and behaviorists are on call 24/7 for any medical or behavioral concerns that arise outside scheduled visits.

Family Partnership

Families are essential partners in our care model. We maintain open communication through regular check ins, progress reports, and care plan meetings.
